The goal of a culture of safety is to “minimize the risk of harm to patients and providers through both system effectiveness and individual performance”

The two competencies I chose to discuss that promote quality patient care in the healthcare organization are quality improvement and safety. These two competencies are very important because according to (Sherwood and Barnsteiner, 2017) preventable harm from healthcare is the third leading cause of death in the United States. (Sherwood and Barnsteiner, 2017) state the deaths from preventable harm are between 251,000 and 440,000. Quality improvement measures are used along with safety to help prevent these circumstances.

The goal of a culture of safety is to “minimize the risk of harm to patients and providers through both system effectiveness and individual performance” (Sherwood and Barnsteiner, 2017, p. 155). According to (Sherwood and Barnsteiner, 2017) errors and problems with safety can occur in all areas of healthcare. As stated by (Sherwood and Barnsteiner, 2017) everyone should be held accountable for providing safe and quality care. Although (Sherwood and Barnsteiner, 2017) put emphasis that it is more important to find out what happened to cause the safety problem instead of blaming someone for it. Finding out what happened is more important because that information can be used to prevent it from happening again.

Quality improvement is what is used to reduce and prevent errors and safety problems from occurring. According to (Sherwood and Barnsteiner, 2017) quality improvement is “using data to monitor the outcomes of care processes and use improvement methods to design and test changes to continuously improve the quality and safety of health care systems” (p. 110) According to (Sherwood and Barnsteiner, 2017) data is gathered from patients, provider and systems. The data that is collected is then used to set in place new protocols and safety checks to provide safe and quality care for patients.
