You dont need any references or front page. The patient you will be writing about has a language barrier. He can’t talk English (he is Chinese), he is on fluid restriction of 2000ml per day, he is also bed bound and needs assistance with turning and he was independent prior to hospitalization. He is also incontinent and dependent to feeding (needs help, can’t feed himself) and has problems swallowing so everything including food and meds must be crushed and cut up really small. He also has a right middle lobe pneumonia, lund lesions. Some thing you can maybe talk about and include in the reflection is listening to lung sounds and helping feed him and monitoring his fluid intake very carefully and making sure he is re positioned every 2hrs. A goal you could also include for next time is finding better ways to communicate with the patient since he has trouble talking and can’t speak English.
