The aim of this assignment is to enable you to devise a specific anticipatory care
plan for one health or social care context.
Profile and judge the need for anticipatory care. Provide a rationale and devise an
anticipatory care plan to respond to a need in a chosen context. Evaluation of the
strategies employed in assessing the impact gains must be made explicit.

The questions that you need to consider is have you addressed the requirements of
the assignment. You have mentioned SLE and outpatient clinics but you need to be
much clearer about your profile choice. You could develop further the case for ACP
in people with chronic conditions. The need for being explicit for the reasons for
ACP is paramount. This is more than about planning for when the persons loses
competence to make a decision. A clearly defined link between ACP and its use in
chronic disease management is required and then you can use SLE as an example.
Remember this is about AC and not an essay on SLE.
