INTERPRETATION OF LUNG AND THORAX ISSUES IN CLIENTS WITH SPECIAL CONSIDERATIONS

 

Scenario: A 28-year-old female undergoes a successful surgery to repair a badly broken arm. Post-operatively, she is in more pain than she expected and says it hurts to breath deeply and has been taking noticeably shallow breaths. Even once the arm pain has been controlled, she says it is still hard to take a deep breath. On physical exam, her trachea appears mildly deviated to the left, and percussion on the left lower lobe is dull (the other lobes are resonant). She has no history of respiratory or cardiac problems. Breath sounds are absent in the left lower lobe and normal elsewhere. Scenario Questions: 28-year-old female What important post-operative nursing care could have prevented this case of atelectasis? What other possible symptoms might a patient with atelectasis have? Also, identify two nursing diagnoses that could be derived from the data in this case study. Are the data in the case study supportive of the diagnosis “ineffective breathing pattern,” or would some other NANDA label be a better fit for this client? What is the definition of the correct NANDA label? Write a nursing diagnosis that begins with the NANDA label you chose, the risk or related factor. and the defining characteristics that led you to select that label. Provide examples of two SMART goals you and the client would select together and three priority evidence-based SMART nursing interventions for this client’s plan of care.
