Describe one innovative health care delivery model that incorporates an interdisciplinary care delivery team. How is this advantageous to patient outcomes?
The primary objective of any health care delivery model is to deliver best patient care at affordable prices by lowering the care expense but still maintaining the standard service quality. An interdisciplinary health care delivery model has a team of an administrator, nurse and a general medical practitioner. They are responsible for giving excellent patient care and maintaining patient service quality. In this model the Nurse department is led by a Patient care manager under unit council with R & D support staff that work relentlessly to keep abreast with innovations in nursing care practices and keep on including best ones in the system. They believe in continuous care and the fees for patient care are paid on participation basis by Medicare and Medicaid services, US health care financing agency. This program is working with association of government approved health centers who claim that they can make difference with this model in patient results. Advantage to patients: Government is helping them as lot many American population will be able to get patient care at affordable prices. Retail clinics: It forms a major part of healthcare system. Both government and private are partnering to provide fundamental services readily to some specific recurring infections like treat flu and throat problems etc. with no delay due to fixing appointment. Advantage to patients: This could make hospitals less crowded as for small ailments people will go to retail clinics.
