The community nurse’s role in helping the elderly patient and family in the home is to coordinate care with multidisciplinary teams to optimize the care that the patient receives. Having great communication skills is imperative because often the nurse will be required to be on the phone discussing those needs with insurance companies, home care agencies, and physicians’ offices (Lancaster, 2019). One barrier to initiating home care services in rural areas is the lack of insurance or being underinsured. Having enough providers in the rural setting is also a setback.

Community health nurses play an essential role in helping families take care of the elderly at home. They serve to help the elderly continue their independent community life for as long as possible by emphasizing preventive care. Community health nurses reduce the burden on family caregivers by taking more care of the sick and elderly in their homes and communities. As they care for patients at home compared to hospital or office environments, home care services in the community will result in more extended visits, which will reduce the number of patients that medical professionals can take care of per day. It is also difficult for medical professionals to include home-based treatment programs in some areas where they are naturally reluctant to visit homes in areas with high crime rates. Also, there are many barriers, such as environmental risks such as infection prevention and hygiene, trouble managing patient autonomy, and risk.

As for well-being, the rural population is consistently less affluent than the urban population. However, the deviation between the two groups is not necessarily large. Rural society is more likely to indulge in unhealthy health-related activities and experience more chronic diseases and exercise restrictions. Therefore, they are at risk of failing to access affordable home health services properly. The nursing theory that most resonates with me is Watson’s theory on caring. Watson’s theory on caring focuses on patient-centered care and bringing a holistic approach to health care delivery. Watson believed that fostering a trusting relationship with the patient and promoting a teaching-learning environment with the patient is fundamental to health promotion (Finkelman, 2017 .I closely identify with Watson’s theory because I believe that nurses treat the person as a whole, not just their disease. Building a trusting relationship with the patient is very important because I have learned from my experience that patients will be more receptive and compliant to treatments if they trust their nurse. I’ve also learned that the more I talk to the patients, the more confident they become in making their own healthcare decisions. This philosophy has guided me to emphasize communication with my patients and their families as I like to focus on the patient rather than just their disease. The nursing theorist that I most closely identify with is Madeleine Leininger since her Transcultural Nursing Theory is relatable in the nursing profession. In this theory, we see that the universal care practices of nursing are based on the culture, beliefs, values, attitudes, and the general practices of an individual or groups who have either similar or different cultures. The aim here is to offer nursing care which is culture-specific so that the people’s well-being can be promoted and adverse human circumstances like diseases can be met in a culturally relevant way. This entails studying and appreciating the diverse cultural backgrounds when it comes to universal nursing care practices, values, and beliefs to ensure the implementation of efficient and effective health care programs in line with the people’s culture. It emphasizes also that different cultures have varied and unique healthcare habits, sickness and wellness values, attitudes, and patterns of action (Finkelman, 2017). To provide all patients with the best possible care, the nurses should be aware of the illness conditions and care needs of their patients. However, this alone is not enough.
