Technique Identifier: Client discharge planning- Case Video : Video Transcript (NOISE – Knock on door) INEZ: Good morning, Mr. and Mrs. Halvorsen. I’m Inez, I’ll be the nurse caring for you today. How are you? (NOISE – Phone ringing) Uh, what a busy day. Excuse me. Hello? Now Graca, mama has to go back to work. You be sure to bundle up for school. I’ll see you when you get home. Yes, yes, I know. We’ll see about new sneakers when I get home from work. Bye for now. INEZ: Okay. I have some discharge paperwork to go over with you. You will be discharged home in the care of your husband. Who else is available to assist you once you get home? MRS. HALVORSEN: Oh, my daughter will stop by in the evenings on her way home from work, as she does every day. And our neighbors are really good. If we need anything, we can call them. MR. HALVORSEN: Now, somebody mentioned something about a nurse stopping in and maybe helping her with a bath a couple of times per week. INEZ: Yes, that was Connie, our social worker, who discussed having a bath aide come to your home twice a week. Here is some information about those services, along with the list of their fees. MR. HALVORSEN: Fees? No one said anything about any fees. INEZ: I know exactly what you are going through, I had that same experience with my aunt Jennie. MR. HALVORSEN: It says here that we have to pay for the full service. There’s not even a senior citizen discount. I don’t understand. All we’re asking for is help with a bath a couple of times a week. INEZ: It’s unfortunate, but Medicare only covers home health services for clients that are homebound. MR. HALVORSEN: My wife doesn’t drive, she can’t get out by herself. We do go out for drives and to attend church. But I don’t think I’m going to be able to give her a bath by myself. INEZ: Why can’t your daughter help out when she stops by in the evening? MRS. HALVORSEN: Oh, she is much too busy to fuss with giving me a bath. INEZ: Let’s discus your care needs, and I will call Connie the social worker to ask her to come talk to you. There might be other options for you. MRS. HALVORSEN: We would like that.
