Answer: the patient’s nurse physician and respiratory therapist collaborated with the patient’s pediatrician and patient’s mother to plan the patient’s post discharge care. the social workers were also included. family teaching was included and the parent was provided education on how to monitor the patient for signs of respiratory distress.

The patient’s parent was kept informed and made a part of the decision making. also, the use of telecommunication was used. What could have been done better would be using video/telehealth during the initial conversation between the provider and nurse for the provider to do a visual virtual assessment of the patient.
