For this discussion I have selected Chronic Kidney Disease (CKD). Kidney Failure or End Stage Renal Disease is a significant public health challenge we face in America today (Healthy People 2020, 2017). The disease contributes to pain and suffering in the populations it affects, as well as, extensive healthcare cost to treat and manage the disease. CKD uses nearly 25% of the Medicare Budget to assist these patients in treatment (Health People 2020, 2017). Health determinants influence health status and are related to many different factors. I will identify some of these and define the following determinants related to CKD.

Genetic determinants play a large role in the development and progression of CKD, this we cannot change. Here in Rural Nebraska we have lots of farmers and ranchers whose diets are high in protein, as they produce eggs, chicken and beef. This may or may not contribute but diets high in protein have been show to predict faster progression of kidney disease, especially in diabetics. Social Factors and physical conditions may be reflected in the way people are born, live, work, play, and age (Healthy People 2020, 2017). Screenings for CKD are not recommended for asymptomatic adults as there is insufficient evidence to support screenings (U.S. Preventative Services Task Force, 2012).

Health Service is a determinant related to access to health services. Studies have been shown that using drugs that reduce proteinuria can slow the progression of kidney disease. By seeking medical attention for annual physicals this could be caught early, managed, and treated with medications to delay progression. This is all determined by availability of services to the patient, cost, insurance coverage, and education to promote awareness.

Individual Behaviors may include monitoring blood pressure and blood sugars to maintain within normal limits reducing the risk for CKD. Compliance with behavior or lifestyle modification programs may improve outcomes related to the development and progression of the disease.
